GENERAL DESCRIPTION OF DUTIES AND
Responsibilities:
The Licensed Practical Nurse works in conjunction with a medical provider, to provide professional, team based medical care and nursing support in a clinical setting.
Under clinical supervision of the Registered Nurse or Provider and operational supervision of the Chief Operations Officer (COO), the nursing staff plays a critical role in the delivery of quality, patient- centered medical care within the context of a federally (HRSA) qualified community health center.
(FQHC).
SPECIFIC DUTIES AND
Responsibilities:
A.
1.
Communicates with patients in a courteous and professional manner in face to face encounters and via the telephone2.
Educates the patient and family member on the patient's diagnoses' and health care self-management, within scope of knowledge and authority.
B.
1.
Participates with the Provider in the implementation of the plan of care and multidisciplinary patient education.
2.
Promotes the Team-based Care Model by participating in daily huddles and bi-weekly PCMH team meetings to communicate and collaborate on patient issues and needs.

C.
1.
Conducts patient screening for hearing, sight, vital signs, blood pressure and other routine procedures.
2.
Assist the Providers with in-office test and procedures as ordered.
3.
Administers medications including IM and Sub Q injections as ordered by the Provider.
4.
Documents clinical activities and patient interactions in the Medical Record.
5.
Collects specimens from patients, performs in-house laboratory testing, and processes specimens and requisitions for transport to the outside testing facility.
6.
Stocks exam rooms and special treatment rooms with needed supplies and indicates necessity for ordering supplies on the appropriate supply order form.
7.
Demonstrates enthusiasm and commitment toward continuous learning and developmental opportunities.
8.
Returns patient phone calls promptly.
9.
Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary.
10.
Functions as a triage nurse.
11.
Interacts with other agencies/medical entities in regards to patient medical information and documenting referrals as necessary.
12.
Participates in education of nursing students as assigned.
13.
Other clinical duties and responsibilities as assigned by the lead clinical person or COO.

14.
Physical attendance is an essential element of the job and necessary to perform the essential functions of the LPN position.
D.
1.
Attends and participates in JHC and other meetings as required.
2.
Will supervise nursing and support personnel as assigned.
E.
1.
Attends all nursing staff meetings and participates in the JHC QI program.
F.
1.
Attends all mandatory OSHA infection control trainings and follows general OSHA guidelines for infection control, including Universal Precautions.
2.
Abides by all JHC infection control policies, including acute response measures in the setting of a public health concern or disease outbreak.
G.
1.
Performs Annual Skills Evaluation and signs the Nursing Competency Skills Checklist.
H.
1.

2.

The Licensed Practical Nurse reports to, is supervised by, and is evaluated by the Chief Operations Officer with input from clinical personnel.
The Licensed Practical Nurse may be asked to supervise a Medical Assistant.
The evaluation of work performance will be on-going, and will be carried out by the Registered Nurse or Provider in conjunction with the COO.

It will include the specific duties and responsibilities of this position description plus employee attitude and general working behavior.
Formal evaluation will normally be performed annually, but may be initiated at other times by the Licensed Practical Nurse or the Registered Nurse or Provider in conjunction with the COO.
1.
Must be licensed with the Virginia State Board of Nursing with a current LPN license.
2.
Must have graduated from an accredited nursing program.
3.
Ability to work a flexible schedule.
This position is a FLSA Non-Exempt position.
This is an OSHA high risk position.
This is an ADA negotiable position.
This position is a Grade C 4.
The salary for this position is negotiated between the individual and the Director of Human Resources and/or the Chief Executive Officer.
